Chapel Hill Middle School is lucky to have our very own Nurse Smith.  If you ever have a question she is there for you.

**IMPORTANT  INFORMATION**

REMEMBER: By law you are required to be up-to-date on your child's immunizations, whether you receive a friendly reminder or not.

Please remember to update your child's immunizations this summer.  If you are delinquent, your student WILL NOT BE ABLE TO PICK UP THEIR FALL SCHEDULE.

Please mail or fax updated records to the school your child will be attending.

Where should you go for immunizations? 

Contact the Health Depart Immunization Branch @ 903-510-5604

You will be required to bring your current shot record.

The cost is $8 per vaccine or you can file on your medicaid.

 ATTENTION:  Students entering 7th grade for the 2010-2011 school year.  You will be REQUIRED to show proof of the following three immunizations before you will be allowed to pick up your schedule in August.

1. Tdap - Tetanus, Diptheria and acelluar Pertussis
2. Varicella #2 - unless you have documented proof of chicken pox disease
3. Meningococcal
